At BioMetrics, one of our goals is to make the process of obtaining prosthetic or orthotic services as easy as possible. For your convenience, we have posted on our website a complete BioMetrics patient form package that needs to be completed at your first visit. By clicking on the link below, you can view and print the entire packet. This allows you to review and complete the forms at your leisure, and spend less time attending to paperwork during your office visit.
The packet also includes a checklist so you can be sure to bring along your completed forms, other documentation, and any items you may need.
